QI Practice Lands Tool to Measure and Manage Care Gaps

QualityImpact member, Shreveport Internal Medicine, has been highlighted in AMA's Wire blog for their ability identify and close care gaps. The practice has increased risk-level identification and care-gap identification by 97% and 77%, respectively. They have been able to dramatically improve control rates because of their ability to target specific measures.

The Consortium for Southeastern Hypertension Control (COSEHC) receives CMS Transforming Clinical Practice Initiative Award

The Consortium for Southeastern Hypertension Control, headquartered at Wake Forest University Baptist Medical Center, is one of 39 health care collaborative networks selected to participate in the Transforming Clinical Practice Initiative, announced today by Health and Human Services Secretary Sylvia M. Burwell. The Consortium for Southeastern Hypertension Control will receive up to $ 15.8 million to provide technical assistance support to help equip clinicians in the Southeastern network region with tools, information, and network support needed to improve quality of care, increase patients’ access to information, and spend health care dollars more wisely.
